Mission As a team oriented practice we are committed to providing extraordinary service and care. In getting to know each of our patients, we are best able to develop individual treatment strategies for long term dental health. We understand the importance of our family of patients and strive to ensure their trust and confidence. First Visit Please bring the following with you to your first visit: Patient questionnaire, both sides completed and signed Insurance form, completed and signed So that we may properly evaluate your dental needs, your first appointment will normally consist of a thorough exam and necessary x-rays. A treatment plan will then be recommended for your particular needs. Payment We request payment at the time services are rendered. For your convenience, MasterCard and Visa are welcome. When necessary, our staff will make specific financial arrangements with you. A finance charge of 1.5 percent per month is added to past due accounts. Insurance As a courtesy to you, we will submit fees for services rendered to your insurance company. However, we do consider the patient primarily responsible for the account. Co-payment (i.e.: the portion your insurance company does not pay) is due at the time of service. On a monthly basis, you will receive a statement reflecting the status of your account.
